Bone Spreading and Condensing System
Split-Control is the minimally invasive alternative to osteotomes. Bone Spreading and Bone Condensing with special screw-like instruments (spreaders) achieve a controlled and standardized dilatation of horizontally resorbed bone and a gentle densification of cancellous bone. The alveolar ridge can be optimally prepared for the subsequent process of implant insertion while maintaining the existing bone substance without complicated horizontal or vertical bone grafting.
Advantages
- Suitable for preliminary implant osteotomy preparation of all threaded implant systems
- Ideal for type D2–D4 bone
- Controlled dilatation of horizontally resorbed bone
- Gentle densification of cancellous bone
- Provides primary implant stability through bone condensing
- Minimally invasive alternative to osteotomes

Step 1Elevate a full thickness mucoperiosteal fl ap to expose the underlying bone. When bone spreading the initial bur prevents the pilot bur from slipping. |

Step 2Use the pilot bur at a drill speed of 1000-1500 rpm. The pilot bur is indexed with markings to correspond to the various implant lengths and is used to create a site of the appropriate depth for the implants to be placed. |

Step 3If more than one implant site is to be developed, a crestial relief cut with the osteotomy disk facilitates bone spreading. Use the osteotomy disk at a drill speed of 800-1000 rpm. |
Step 3In soft bone (D3 and D4), prepare the osteotomy site with the 1.8mm expansion bur to the appropriate implant length. For more dense bone (D1 and D2) it may be necessary to expand the osteotomy with the 3.0mm expansion bur to the appropriate implant length. |
Step 4Bone spreading is achieved through the progressive deployment of a series of spreaders with increasing diameters. The spreaders are graduated in .2mm increments, starting at 2.7mm at the crest. In D1 and D2 bone, it may be necessary to widen the osteotomy with the expansion burs prior to deploying the spreaders. To facilitate insertion, the carrier and driver are provided in the surgical kit. |
Step 4Once the appropriate osteotomy is created with the expansion bur, begin condensing the osteotomy using the bone spreaders (refer to Step 4 in the Bone Spreading Technique) |
Step 5The implant is now placed in the osteotomy. |
